Patient Room Turnover Checklist for Environmental Services Teams

When a patient is discharged, the clock starts. The next patient may already be waiting in the emergency department. Environmental services (EVS) teams are under constant pressure to turn rooms quickly, and the room still has to be cleaned and disinfected properly, because the surfaces in a patient room can spread infection to the next person.

This checklist reflects common practice for discharge (terminal) cleaning. Your infection prevention team's protocols, product instructions and isolation precautions always take priority. The CDC's guidance on environmental cleaning in healthcare settings is a widely used reference.

Before Entering

  • Check for isolation signage and use the required personal protective equipment
  • Confirm the patient and belongings have left the room
  • Gather supplies: approved disinfectant, cloths, mop, bags, fresh linens
  • Perform hand hygiene and put on gloves

Remove and Discard

  • Remove used linens and bag them according to policy
  • Empty bins and replace liners
  • Remove disposable items and leftover supplies according to policy
  • Dispose of sharps only in sharps containers; never handle loose sharps

Clean and Disinfect: High-Touch First

Work from clean to dirty and high to low.

  • Bed rails, controls, call button and over-bed table
  • Light switches, door handles and cabinet handles
  • Telephone, TV remote and chairs
  • IV poles and equipment surfaces as assigned
  • Sink, taps and dispensers

Follow the disinfectant's contact time: surfaces must stay wet for the time on the label to be effective.

Bathroom

  • Toilet, seat, flush handle and grab bars
  • Shower, floor and walls as needed
  • Sink and fixtures
  • Restock toilet paper, soap and towels

Floor and Final Steps

  • Dust mop then damp mop the floor
  • Make the bed with fresh linens
  • Restock standard room supplies
  • Remove gloves and perform hand hygiene

Inspect and Release

  • Walk the room for missed areas
  • Use any verification method your facility requires, such as fluorescent markers or ATP testing
  • Mark the room as clean in the bed management system
  • Note any maintenance issues: broken equipment, damaged surfaces

Isolation Rooms

Rooms for patients with certain infections, such as C. difficile, may require specific disinfectants and procedures. Always follow isolation protocols, and never skip steps to save time.

Turnover time matters for patient flow. Improve it by:

  • Getting discharge notifications quickly
  • Prepped cleaning carts with everything needed
  • Clear priorities when several rooms need turning at once
  • Tracking turnover times to spot bottlenecks
