Assisted Living Night Shift Checklist

Night shifts in assisted living and residential care run with fewer staff, residents who may be confused or restless, and the expectation that anything can happen at 3am. The overnight team is responsible for safety, dignity and a calm handover to the morning shift. A clear checklist helps tired people do the important things in the right order.

This checklist covers general overnight routines. Clinical procedures, medication administration and staffing ratios are set by your state or national regulator and your organisation's policies, and those always take priority.

Handover From Evening Shift

  • Get a verbal and written handover for every resident
  • Note residents who are unwell, at high fall risk, new, or recently returned from hospital
  • Confirm any changes to care plans, diets or medications made today
  • Check which residents need repositioning, continence care or checks at set times
  • Confirm who is the nurse or senior on call and how to reach them

First Round

  • Check every resident according to their care plan
  • Make sure call bells are within reach
  • Check bed rails, sensor mats and alarms are in place where care plans require them
  • Ensure walking aids are within reach and floors are clear
  • Check room temperature and lighting, including night lights

Fall Prevention

Falls are one of the most common and serious incidents in care settings, and many happen at night when residents get up to use the bathroom. The CDC's STEADI resources on older adult fall prevention are a practical reference.

  • Keep paths to bathrooms clear and lit
  • Respond quickly to call bells and sensor alarms
  • Offer toileting support at scheduled times
  • Make sure residents have appropriate footwear nearby
  • Report and document every fall, even without injury

Through the Night

  • Complete rounds at the intervals your policy and care plans require
  • Log checks with times, not a single entry at the end of the shift
  • Support residents who are awake or distressed, calmly
  • Monitor residents who are unwell and escalate concerns promptly
  • Keep corridors, fire exits and stairwells clear

Building and Safety

  • Check doors and windows are secure and alarm systems are on
  • Know the fire evacuation plan, including residents who need assistance
  • Check emergency lighting
  • Know where the emergency contact list, first-aid supplies and AED are

Documentation

  • Record care given, observations and any changes in condition
  • Complete incident reports for falls, injuries or behavioural events
  • Record times accurately
  • Write clearly enough that the day team can act on it

Morning Handover

  • Prepare a written summary for each resident with anything notable
  • Hand over verbally, highlighting residents who need attention
  • Pass on maintenance issues and supply shortages
  • Confirm all incidents have been reported to the right people
